Elevate

Elevate is a local campaign founded to help adults in northeastern Otter Tail County age well. Elevate partners with area organizations to both promote existing and create new opportunities for residents to improve their physical, mental, and social health. Whether someone is needing help eating well, staying active, making connections, understanding their medicare plan, and more. Elevate can connect them with the information or resources they need to optimize their health. Perham’s Elevate program is a grant-based pilot campaign awarded to Perham Health and funded by Leading Age Minnesota Foundation and UCare Minnesota. One of only three such initiatives in the state. Elevate was founded 2022 to address the expected increase of healthcare concerns for a growing populations of adults.
