LB Homes formed over 100 years ago as a church mission to care for the elderly. 100 years later LB Homes continues that mission of providing Christ-centered care and support to seniors living in and around the Fergus Falls area.  Whether it is long term nursing care at LB Broen Home, at home care with LB Home Care, or independent living at our LB Sheridan House, we make it our mission to care for you, wherever you call home!

BROEN HOME | SKILLED NURSING CARE

Our nursing home has served Fergus Falls and surrounding communities since 1959 by providing exceptional long term care for older adults. We offer a variety of care services, such as help with daily living essentials and hygiene. Our skilled nursing staff oversee the personalized care for each resident to ensure quality and safety. Combined with spacious private rooms and friendly atmosphere, we make our residents feel right at home.

We strive to meet the specific care needs of each resident. For instance, a resident may need help managing advanced chronic conditions, or they may require longer recovery time after an injury or illness. With any situation, we provide excellent customer service by making the needs of our residents and families a priority. INDEPENDENT LIVING AT SHERIDAN HOUSE

Friendly people in a caring environment is what you get when you choose independent living at LB Sheridan House. We offer one and two bedroom apartments with optional services for older adults who want to reside in a community of peers and continue to live a fulfilling life. LB Sheridan House is part of the LB Homes campus (including LB Broen Home and LB Short Stay), which provides a wider sense of community and a variety of opportunities to have fun and socialize with friends. Our Light and Life Auditorium is a beautiful venue for Sunday worship services and for all kinds of events and social gatherings from children’s music recitals to family reunions. LB HOME CARE

LB Home Care helps individuals who want to remain living independently and safely at home. Our focus is to provide quality services that help you reach your best possible level of function and keep you actively participating in community life.

Taking advantage of our home healthcare services can help reduce medical costs with less time in the hospital and quicker recovery time after hospitalization. People of all ages benefit from LB Home Care, including those who:

• Received a new diagnosis or complication of a chronic illness
• Were recently discharged from a hospital, nursing home, or short stay rehab unit
• Are recovering from surgery or major illness
• Need medication management and education
• Require assistance with bathing
• Need support for wound care or IV therapy (for antibiotics, hydration, or intravenous nutrition)

Care Services

Our goal is to keep you at home longer with your loved ones and avoid rehospitalizations and emergency room visits. Our interdisciplinary team is in regular contact with a patient’s primary physician to receive orders and /or directions for your care. Our staff provides a wide range of servcies to meet your home health care needs:

Nurses – Our nurses assess and monitor patient health as well as provide valuable education and exceptional care at home for:

• Managing illness
• Infection Control
• Wound care
• Home safety
• Certain Medication management services 
• IV therapy

Therapists – We offer physical, occupational and speech therapy to improve and restore skills that were lost or diminished due to illness or accident. After discharge from our LB Short Stay, patients can request further rehab/therapy services through LB Home Care and continue working with the same therapists at home.

Home Health Aides – Under the supervision of a registered nurse, our home health aides provide periodic personal care, light meal preparation and housekeeping, and give verbal or visual reminders for performing scheduled treatments and exercises.

Depending on medical diagnosis and needs, Home Health Care services may be covered by private insurance, Medicare and Medicaid, major commercial insurance, HMO, PPO, and self pay.  

We are a Medicare Certified Home Care Agency.

TRANSITION CARE | LB SHORT STAY

Stay with us to recover and regain your strength after a surgery, injury or major illness. Our highly trained therapy staff on-site at LB Short Stay delivers rehabilitation services allowing you the recuperation you need to make your transition from hospital to home go smoothly. In the spirit of teamwork, one of our five core values, our interdisciplinary team makes sure every medical professional involved in your care is on the same page. We’re here to get you well and guide you, so you stay healthy and avoid future trips to the emergency room. In fact, our re-hospitalization rate following discharge is significantly lower than state & national averages.

Our LB Short Stay services can benefit all types of situations. Here are just a few of the reasons people stay with us to receive the care they need:

Care Services

Our interdisciplinary team of medical professionals ensures your up-to-date medical information is communicated consistently to everyone involved in your care, including your doctor. We offer:

• 24-hour skilled nursing care to give you the support you need around the clock
• Personalized care coordination with comprehensive evaluations to create an appropriate, effective plan of care
• On-site therapy department to provide occupational, physical and speech therapy
• Intravenous (IV) care to safely carry out doctor orders regarding fluid and medication delivery
• Wound care with the optional use of a wound VAC (vacuum assisted closure) if needed
• Rehabilitation/Therapy Services

Rehabilitation/Therapy Services

Our certified therapists from Aegis Therapies provide physical, occupational and speech therapy. They have extensive knowledge and receive specialized training unique to LB Homes. Our therapy department is on-site, so you get the rehabilitation you need without having to travel outside the facility. Whether you need inpatient or outpatient care, our warm surroundings and friendly atmosphere allow you to recover in peace and comfort. We offer:

• Physical Therapy to bring back strength and range of motion. Our Doctor of Physical Therapy on staff brings a high level of expertise and oversees the physical therapy plans for our patients.
• Occupational Therapy to practice day-to-day functional tasks. We have specialists certified in GEM (Geriatric Evaluation & Management) as well as therapy involving lymphedema.
• Speech Therapy to work on breathing and speaking techniques. After an injury, our speech therapy staff can also work with you to restore cognitive performance or even swallowing functions.
• Available for both Inpatient & Outpatient Care

And, if you need to receive additional rehabilitation/therapy services following your stay with us, you can request LB Home Care and the same team of therapists from LB Short Stay will continue working with you at home.
